At Relay, we welcome you back if you had to leave Relay for some period of time and now want to return to the program.
In order for Relay to evaluate your re-entrance request, you must send us this completed form with an attached letter explaining your request. Before submitting this form, be sure to complete the following steps:
1. Contact your Dean’s Office. Ensure you fully understand the academic implications of returning and/or transferring programs and/or campuses at Relay before submitting this form.
2. Contact Student Services. Ensure you fully understand the financial aid and/or financial requirements of your request.
The Office of Enrollment Services, upon receiving your form, will contact the appropriate Dean’s Office and the Student Financial Services to confirm that your return has been approved. The Enrollment Services team will send you a confirmation e-mail once your request has been processed.
