The partner hour verification form is sent to your school by Relay's AmeriCorps team as a part of closeout (usually in April/May). The form is required and completion confirms your service hours for the term.

Your school must complete the partner hour verification form and you must complete all close out tasks before you can be exited from the program.

Full-time Relay AmeriCorps members must serve a minimum of 1700 service hours to complete the program and earn the Segal AmeriCorps Education Award. Teachers should easily acquire the minimum requirement through their daily activities and Relay program requirements.
